Where I work there are design safety reviews conducted, but there is a catch. The safety of a particular design is not reviewed until the design causes an injury. While this is not the most effective way to do things, it is the Royal Oak way. More often than not we find ourselves being reactive to situations instead of being proactive in prevention. I would have to say that our design safety reviews are effective in a sense. They are effective in the sense that once an injury or accident has occurred, the hazards of the design are then removed or reduced by the review. I believe that our safety program would be much more effective if we would focus on being proactive instead of only reacting to accidents and injuries.
